Chronic care management · Sarasota area

Someone watching out for residents every month, so small problems are caught before they become ER trips.

ClearPath gives assisted living residents with ongoing health conditions a dedicated care team between doctor visits. We work alongside the physicians who already care for them, at no cost to the community or its physicians.

  • Covered by Medicare Part B
  • Residents keep their own doctors
  • Stop any time
What residents get

Steady support between doctor visits

Chronic care management is a Medicare program for people living with two or more ongoing health conditions. Here is what every enrolled resident receives.

A familiar face

The same care coordinator each month, under the direction of our nurse practitioner.

A personal care plan

Goals, medications and what to watch for, covering all of their conditions, with a copy for them.

Medication help

Refills, pharmacy questions and making sure medicines are taken as prescribed.

Early warning

Changes in symptoms, weight, mood or falls are passed to a clinician the same day.

Coordination

Doctors, specialists, pharmacies, labs and community staff stay connected and informed.

Back home safely

After an ER visit or hospital stay we gather records, check medications and set follow-ups.

Appointments and services

Scheduling, transportation, medical equipment and preventive care that is due.

24/7 access

A member of the care team can be reached day or night for health questions.

Behavioral health support

Care for mood and well-being, too

Depression, anxiety and loneliness are common in later life, and they can make every other condition harder to manage. For residents who need it, ClearPath adds monthly behavioral health support alongside their chronic care.

  • Monthly check-ins on mood, sleep and well-being, using simple, standard questionnaires
  • A behavioral health care plan with goals the resident helps set
  • Coordination with their therapist, counselor, prescriber and community staff
  • Quick follow-up from our nurse practitioner when mood or behavior changes

How it works

Behavioral health support is optional and has its own sign-up. It is covered by Medicare Part B, and standard coinsurance may apply. Residents can join chronic care, behavioral health support or both, and stop either one at any time.

In a crisis?

If someone is thinking about suicide or in emotional crisis, call or text 988 to reach the Suicide & Crisis Lifeline any time. For an emergency, call 911.

How it works

Four steps from first visit to steady care

  1. 1

    Introduce

We meet with the community's leadership, then share information with residents and families.

  2. 2

    Choose to enroll

Our nurse practitioner meets each interested resident. The resident or their representative decides whether to join.

  3. 3

    Monthly care

A dedicated care coordinator checks in every month and handles the follow-up that keeps care on track.

  4. 4

    Everyone informed

Physicians and families get updates, and urgent concerns go to the right clinician the same day.

For residents' physicians

Your patients stay your patients

We work between your visits and report back to you. There is no cost to you and nothing to sign.

How it helps you

  • Monthly updates on what happened between visits: symptoms, falls, ER trips and medication changes
  • Fewer routine calls to your office about refills, appointments and records
  • Patients arrive with medications reconciled and concerns already flagged
  • Prompt follow-up after hospital and ER discharges

Our boundaries

  • We never replace you as the resident's physician
  • We never change your orders or medications. Any concern is sent to you.
  • We never enroll a resident you already provide chronic care management for. We check first.
  • We never ask you to pay, sign a contract or change how you practice

Prefer to provide chronic care management to your own patients? Tell us, and we step aside for them.

For residents and families

Clear, voluntary and on your terms

Joining is always the resident's choice. They keep their own doctors, and they can stop at any time.

Who can join

  • Has Medicare Part B
  • Lives with two or more ongoing conditions, such as diabetes, heart disease, COPD, arthritis, kidney disease or memory loss

Chronic care management is covered by Medicare Part B. Standard Medicare coinsurance may apply and is often covered by supplemental insurance.

About our logo

Two paths rise and meet in a heart. The paths are the guidance we give between doctor visits: medications, appointments and follow-ups kept on track. The heart is why we do it.

What it costs the community and physicians

Nothing. ClearPath is paid through Medicare for the care we provide to enrolled residents.

Questions

Frequently asked questions

What is chronic care management?

It is a Medicare program for people living with two or more long-term health conditions. A care team supports them every month between doctor visits, keeping medications, appointments and follow-ups on track.

Will my loved one have to change doctors?

No. Residents keep their primary care physician and specialists. We work alongside them and keep them informed.

Does it cost anything?

There is no cost to the community or to physicians. For residents, the program is covered by Medicare Part B. Standard Medicare coinsurance may apply, and supplemental insurance often covers it. We explain this before anyone enrolls.

Who is on the care team?

A licensed nurse practitioner oversees each resident's care. Trained care coordinators handle the monthly check-ins and follow-up, and pass any clinical concern to the nurse practitioner right away.

What is behavioral health support?

It is an optional add-on for residents living with depression, anxiety or other behavioral health needs. Each month we check in on mood and well-being, keep a behavioral care plan up to date, and coordinate with their therapist, prescriber and community staff. It does not replace counseling or therapy; it helps everyone involved stay connected.

Can a resident stop the program?

Yes, at any time, for any reason. Just let the care coordinator or our office know.

What if the resident's doctor already provides this service?

Medicare allows only one practice to provide chronic care management each month. We ask every resident before enrolling, and if their doctor already provides it, we step aside.
